TO REPORT WORK-RELATED INCIDENTS

As of August 31, 2009, Morehead State University’s Workers’ Compensation administrator is Kentucky Employees Mutual Insurance (KEMI). 

The procedure for filing a first report of injury has changed. You will no longer contact Underwriters via a toll free number to report an injury.

If the injury is not emergent in nature, the employee should immediately notify their supervisor and then visit the Office of Human Resources, 101 Howell McDowell to report the injury. If the injury requires treatment in an emergency room, the supervisor should report the injury to Human Resources as soon as possible.

*If unable to immediately report an incident to Human Resources, please complete and send the above form to Human Resources as soon as possible.
