Course Substitution Form - This form is used for transferring work in from another university or for substituting another class for one that is required in your program. 

Course Substitution Form- MAT - Specialized Area - This form should be used if you are substituting courses in for the MAT Middle Grades or Secondary content area.

Change of Name/Address - Must be completed through the Registrar's Office

Criminal Consent Form - This form is used if a student has ever been convicted of a criminal offense other than a traffic violation. Please submit the form to the Office of Enrollment Services at 100 Admission Center, Morehead, KY 40351 or by fax at 606-783-5038.

Early Graduate School Program Form - This form is used for undergraduate students who want to take up to a maximum of 12 graduate credit hours. Please visit the Early Graduate School Program site for program guidelines and more information. 

Major James Scott DeHart Graduate Fellowship Application - Please submit application to the Graduate School at 701 Ginger Hall, Morehead, KY 40351 or by fax at 606-783-5061.

Release of Academic Information - This form is used to request copies of transcripts, test scores, recommendation letters, etc. Please submit the form to the Graduate School at 701 Ginger Hall, Morehead, KY 40351 or by fax at 606-783-5061.

Request to Waive the Time Limitation Waiver Policy - This form is used for students who are transferring in coursework older than 10 years from an external institution.
