2009 Success Academy Application Process

| To apply for the 2009 Success Academy, the application blank, financial aid data sheet, and housing application should all be sent together as soon as possible. If you want to commute rather than living on campus, you still must submit a housing application. |

| Please note: If you do not have two or more ACT subscores below 18 in English, 19 in mathematics, or 20 in reading, you are not eligible for admission to the Success Academy. |

| As quickly as possible after receipt of your documents, you will be notified of your admission to the academy and receive a detailed packet of Academy information on all aspects of the program. |

Click on the links below to open the Success Academy Application, Personal Data Sheet, and Housing Application. All three documents are in Adobe .PDF format. The forms can be filled out on your computer screen, or printed and filled out by hand. After completion, all three forms should be sent together either by mail, or faxed to the Academy director as described below.
