You must submit your completion form and pay your $20 fee by the deadline above. After the deadline, the fee will increase to $30. Please contact the Office of Accounting & Financial Services at (606) 783.2019 to pay your fee. Please notate the receipt on this form or contact the Graduate School at (606) 783.2039.
