These are forms frequently needed for financial aid processing. Please carefully review the instructions and content of the form before completing the form as they are not required for all students.

Student Aid and Information Forms

  • 2021-22 Federal Work-study Waitlist- This form is for students who were not originally awarded federal student employment (federal work study) for fall and/or spring and wish to be placed on a waiting list.
  • 2021-22 Student Request to Adjust Financial Aid Form - For students who wish to reduce/decline student loans or for students who will not be attending and wish to decline all aid. 
  • Concurrent Enrollment Form - For students who are concurrently enrolled at another school and receiving financial aid through Morehead State University. You must contact the Financial Aid Office if you intend to take less than six credit hours per semester.
  • Satisfactory Academic Progress Appeal Form - For students who have been placed on Unsatisfactory Academic Progress and wish to appeal. Students must provide a typed appeal, documentation and a copy of their unofficial transcript with the SAP Appeal Request form. Unofficial transcripts can be printed from the student's account under "Academic Profile."
  • Scholarship Appeal Form - Students may file a scholarship appeal if the academic renewal criteria to receive their scholarship was not met due to extenuating circumstances.  The form must be submitted to MSU Office of Financial Aid by the student and include a typed letter, supporting documentation and an unofficial transcript. The appeal form is available on the MSU website at and should be submitted as quickly as possible upon receiving your scholarship cancellation email.  The student is responsible for payment arrangements with the institution pending a decision of the appeals committee. If approved, students will receive a plan and enter into a contract specifying academic requirements necessary to remain eligible to receive their scholarship for the remainder of their enrollment or until scholarship standards are achieved. Decisions made by the Scholarship Appeal Committee are final and cannot be appealed.
  • Professional Judgment Request Form- According to the Department of Education, a family’s 2019 income is used to assess financial need for the 2021-2022 academic year. If a family’s 2021 income is lower due to special circumstances, the 2021 income may be used to assess financial need.
  • Statement of No Parental Support Form - This form is for students who completed the Free Application for Federal Student Aid (FAFSA) but did not provide their parent's information due to a unusual circumstance(s) or parent(s) refuse to provide financial information on the student's FAFSA.
