- Course Substitution Form - This form is used for transferring work in from another university or for substituting another class for one that is required in your program.

- Early Graduate School Program Form - This form is used for undergraduate students who want to take up to a maximum of 12 graduate credit hours. Please visit the Early Graduate School website for program guidelines and more information.

- Request for Extension to the Maximum Time Limit
- Request to Waive the Time Limitation Waiver Policy - This form is used for students who are transferring in coursework older than 10 years from an external institution.
